How to Make a Referral
Before admission to service, access to prior clinical assessments and other relevant information is requested. All referrals are assessed to determine whether the service user’s needs can be met. A full individualised needs and risk assessment will be completed for each service user and a discharge plan will from part of the pre-admission assessment pack that will be reported back to the referrer within 2 working days. Emergency referrals are also welcome. Where the referral is accepted, an individual Placement Agreement or contract will be completed and signed.
